YOU MUST follow these instructions EXACTLY.

• Click on this link Nomination Form and print the Nomination Form.

• Print your name and phone number on the bottom of the Nomination Form.

• Take the nomination form AND a copy of this page to the auto repair garage you want to nominate. Have THEM fill the rest of the form out and have THEM fax the form to 480-598-3600.
  

Then we will run the shop through a variety of filters and if they pass all of our tests, the shop you nominated will appear on our new list of good auto repair facilities.
